Used in Microsoft Excel, each page is set up to enter information directly and printed. If your version of Word allows, print the worksheet to a pdf file to be emailed to related participants; with a copy put in the Meeting Agenda folder, and a copy put in the Team Director project archive to track incremental progress.
Every time an Action Item Worksheet is assigned, the relevant information is entered into the Action Item Log to track accountability.
Suggested is using the same projector for displaying PowerPoint presentations to display the Action Item Worksheet and fill it out with the participation of everyone to distribute tasking logically and equitably.
As an option if your version of Word does not print individual worksheets in pdf:
By installing the free software PDF995, a printer option is provided that prints to pdf files; so that during meetings the Action Item Worksheets are printed silently and emailed directly to the related participants.
The free version has advertising that delays processing slightly, and the paid version has no delay.
Suggested is a standard policy to send a form consistently to human resources for performance evaluations for any person not providing support for Action Items assigned and agreed upon.
Often times some participants have personality conflicts, social complications to manage, undisclosed competing assigned priorities, health issues, corrupt office practices hindering progress, lack of availability of resources ... and many other potential complications that impede productive progress. If the person is inexperienced in resolving related issues in a timely manner, then notification to the Team Director is appropriate "long before" deadlines are approached.

ALT-Enter in Excel field allows for adding multiple lines of information in a field. Be aware of the effect on the printed form. Potentially adding field lines will extend the form to extend to more than one page. An alternate method to enter a large amount of information in a small text field is label in the top-left corner of an external Word/Excel document. Provide a Work Package number (WP#) sequentially assigned by Team Director to track overall project segment progress; and the Action Item number assigned. Keep a copy of these interrelated documents attached to the related Action Item forms.
